If you have one of these insurances, you must complete additional brief forms that will be emailed to you before your initial appointment.

If your insurance listed above does not have an asterisk, all copays, late-cancellation/no-show fees will be charged by BeNu. As a courtesy, insurance claims will be also be submitted by BeNu.

If your insurance is billed through Headway:

You will receive a welcome email directly from Headway before your appointment.
Before your first visit billed through Headway:
- Confirm your email
- Sign all consent forms
- Add your payment information (credit/debit/HSA/FSA) to Headway’s secure system for insurance copays & certain fees.
Headway will process payments and submit claims to your insurance on yours & BeNu’s behalf.
⚠️ Advisory:
- In addition to Headway, you must also have your insurance information and a valid payment on file with BeNu that will be entered via Tebra, our main electronic health record (EHR).
- Headway is used strictly for billing purposes with the above insurance plans. All appointments and medical information will be found via Tebra (EHR), not Headway.

💳 A credit/debit/HSA/FSA card is required to be securely filed with BeNu. This card will be charged for the payment due the day the service is rendered. Please plan to have funds in these accounts for this charge accordingly. If you need to update your card on file, call BeNu at (904) 544-3608.

Advisory: All new and established patients are billed separately for the Annual Fee as a self-pay fee due to these services not being covered by insurance. Payment is required prior to administering any Creyos cognitive testing. Call/text BeNu at (904) 544-3608 to request to be billed. After payment is received, you will be sent an email from Creyos/BeNu (Dr. Angela White ARNP) with a link to complete the cognitive test(s) from your personal space of choice. ⚠️ All Patients Seeking Evaluation/Treatment for ADHD or Cognitive Impairment:

• If No Prior Diagnosis: You will be required to complete a computerized cognitive test via Creyos testing. If you are a New Patient, we require this test to receive treatment for ADHD. Please see the Required Annual Comprehensive Integrative Fee pricing above for more information.

• If History of Diagnosis: If you have been diagnosed by another provider, you must submit medical records/documentation of the diagnosis/treatment prior to receiving treatment for ADHD. Submitting any previous related testing results is also recommended. If you are unable to provide medical documentation of a previous diagnosis/treatment, you will be required to complete a computerized cognitive test via Creyos testing. Please see the Required Annual Comprehensive Integrative Fee pricing above for more information.

• Autism Spectrum Disorder: BeNu providers are not currently certified to conduct formal standardized testing, such as the Autism Diagnostic Observation Schedule (ADOS-2), to diagnose Autism Spectrum Disorder in children or adults.

Late-Cancellation / No Show Fees:

⚠️ Appointments canceled less than 24 hours in advance or missed without notice are considered late cancellations or no-shows.

• A $75 fee (or up to the full session cost) will apply

• If you are not visible on screen within 10 minutes of your telehealth appointment start time and have not contacted the office at (904) 544-3608, the visit will be marked as a no-show.

• Late-Cancellations/No Shows make it difficult to accommodate other patients who need care.

Administrative Services Fees:

• Excuse Notes for School/Work: No Charge

• Patient Records starting at $35.00 + $1.00 per page after the first 25 pages

• Patient Letters starting at $35.00+ (pending type/complexity of patient history)

• Emotional Support Animal (ESA) / Service Animal Letters: $85.00 annually

• Disability Forms, FMLA Forms, Work Accommodations starting at $150.00 + (pending complexity)

• Peer Reviews starting at $175.00+ (pending complexity)

• Accommodation Letters (e.g., 504, IEP, etc.): $150.00

• Non-controlled Prescription Refill Requests Without an Appointment: $35.00 per prescription (must have provider approval)

NOTE: Controlled prescription refills DO NOT get ordered without a completed appointment - NO EXCEPTIONS
